This Form Solutions installation is operated by the entity identified on the about page. Contact information is on the contact page.

2. Data collection

The only data collected from end users is what the operator explicitly asks for in each form. The operator is the data controller; this software is the data processor. The operator's privacy policy governs what data is collected and how it is used.

3. Use of the service

End users agree to use the service for lawful purposes only. No warranty is provided — the software is provided "as is" without guarantee of fitness for any particular purpose.

4. Data retention

Submissions are retained for as long as the operator chooses. Default retention is indefinite; the operator may configure shorter retention via Settings.

5. Changes to these terms

The operator may update these terms. The "last updated" date above will reflect the change. Continued use of the service after a change constitutes acceptance.
